Patient Guide

Patient's privacy and dignity will be respected at all times. The Hospital goes to great lengths to understand that our patients have different cultures and religions.

 

 Patient's Right & Responsibility

  • Be treated with dignity and respect. Be honest with us; tell us all you know about your present illness, your previous hospitalizations if any and any other matter relating to your health that will help us treat you better.
  • Participate in decisions involving your medical condition.
  • Information and explanation about your medical condition.
  • Know thenames and roles of your healthcare providers. Respect the right of other patients and staff of the hospital.
  • Please ensure that your visitors are considerate of other patients particularly with regards to noise and observe the “Visiting hours” schedule ofthe hospital.
  • Observe the “No Smoking” and other rules of the hospital.
  • Privacy, confidentiality of your records and safety.
  • We would appreciate minimum use of mobile phones as it disturbs others and interferes with medical equipment. We request our patients/relatives to keep their mobiles in silent mode when they are at thehospital. Switch off while entering the ICU.
  • Give us feedback.

 Discharge

  • Hospital room checking out time is 12:00 noon.
  • Post discharge, you may have to process a lot of papers with the insurance companies or with your own organization.
  • It will be prudent if you complete all the formalities at the time of discharge .Your Rmo/ ward sister will give you a discharge summary at the time of discharge. If you should need any additional assistance in this regard, you may contact your consultants during OPD. Please note that the history once recorded cannot be changed.

 Other Useful Information

  • Please bring all your previous medical records.
  • You should not carry any valuable ornaments.
  • For the well being of all the patients, kindly ensure that you limit the number of visitors meeting you when you are hospitalized.
  • Ambulance: You can make request for an ambulance at the reception counter/ nursing counter. Payments should be made to the ambulance service providers only.
  • Please do not make any payments inside the hospital except the cashier, do not hesitate to inform us if you are asked for.
  • Children below the age of 12 years will not be allowed to visit you.
  • While you are in a procedure, we allow a total of 3 visitors in the second floor. Your consultant will call them once the procedure is over.

 Feedback Form

  • We would really appreciate if you could fill in the feedback form provided and put in your valuable inputs before leaving.
  • This will help us evaluate ourselves better and serve patients in a more efficient way. Your identity will not be disclosed to any hospital staff.

 Admission

  • In an emergency, you can get admitted to the hospital any time.
  • In case of Walk-in/ emergency admission the admission to the class desired, is subject to availability of the room/bed.
  • When you come for the admission you will be sent to the bed allotted, one relative will accompany you to the ward and another will wait at the admissions counter to complete the formalities.
  • At the time of admission, you have to take care that all the booking formalities are completed before hand i.e. payment of the deposit and procedure charges.

 Consent Form

  • This is a statutory requirement and is necessary to be signed before any procedure. Your consultant /his assistant will explain you the details of the procedure. You are advised to clear all your doubts with him and then sign the form. There can be more than one form, for a procedure.

 Upgrading of Classes

  • The request of transfer is subject to availability of the rooms.
  • In case of transfer from lower class to higher class, the charges for higher class will be applicable retrospective from the day of admission.

 Final Bill

  • In case you are discharged on Sunday, public holiday or after the billing hours i.e. 11:00am to 8:00pm you can collect the bill on the next working day. We will be taking an approximate bill amount plus a security amount as a deposit before discharge, which will be considered at the time of final bill.
  • The deposits can be adjusted only against the final bills and not with the interim bills.
  • At the time of discharge, we generate the final bill. This will consolidate all the interim bills and the deposits paid.

 Payment

  • The payments can be made in cash / demand draft / pay order. The pay order / demand draft should be drawn in favor of ‘CITY HOSPITAL & RESEARCH CENTRE’.

 Gratuity & Tips

  • Gratuity & Tips to any staff members are strictly forbidden.

 Pharmacy

  • The hospital has an attached 24 hours pharmacy on the ground floor. All medicines and consumables for admitted are to be purchased from the pharmacy.
